A pop during a cutting drill, a knee that swells quickly, and a young athlete suddenly unable to trust their leg can change an entire season. For families facing pediatric ACL surgery, the questions usually go beyond the MRI: Is surgery necessary? Will it affect growth? When can my child safely return to the sport they love?
An ACL tear is a significant injury at any age, but treatment planning is different for children and adolescents. Young athletes may still have open growth plates, have years of sports participation ahead of them, and need a plan that protects both their developing knee and their long-term performance. The right approach begins with a precise diagnosis, a clear discussion of options, and rehabilitation that progresses according to objective recovery milestones rather than the calendar alone.
When Pediatric ACL Surgery Is Recommended
The anterior cruciate ligament, or ACL, helps control rotation and forward movement of the shin bone beneath the thigh bone. It is especially challenged during sports that involve jumping, pivoting, decelerating, or changing direction, including soccer, basketball, lacrosse, football, skiing, and gymnastics.
A complete ACL tear does not always produce constant pain. Some young athletes can walk comfortably once early swelling improves. The more concerning symptom is instability: the knee buckles, shifts, or gives way during a turn, landing, or sudden stop. Repeated instability episodes can injure the meniscus or cartilage, which can make a straightforward ligament injury more complex.
Surgery is not automatic for every ACL injury. A younger child with a partial tear, a stable knee, and low-demand activity goals may be considered for structured nonsurgical treatment with physical therapy and activity modification. However, reconstruction is commonly recommended for young athletes with a complete tear who want to return to pivoting sports, have recurrent instability, or have associated meniscus damage.
The decision depends on skeletal maturity, the pattern of injury, the child’s sport and level of participation, family goals, and the ability to commit to rehabilitation. Waiting may be reasonable in select cases, but continuing high-risk sports with an unstable knee can increase the chance of further joint damage. Why Growth Plates Change the Surgical Plan

If the MRI or arthroscopic examination identifies a meniscus tear, the surgeon may repair it at the same time when possible. Preserving the meniscus matters because it cushions the knee and helps protect joint cartilage over the long term. A meniscus repair can change the early rehabilitation plan, sometimes requiring more restrictions on weight-bearing and knee bending than an isolated ACL reconstruction.
The procedure is generally performed arthroscopically through small incisions, with additional small incisions as needed to obtain and secure the graft. Families should expect a detailed discussion before surgery about anesthesia, pain control, crutch use, school attendance, physical therapy, and the specific limitations that apply to their child.
Preparation Before Surgery Can Improve Recovery
A swollen, stiff knee is not the ideal starting point for reconstruction. Before surgery, many athletes benefit from prehabilitation, often called prehab. This early phase focuses on reducing swelling, restoring full knee extension, improving motion, and rebuilding control of the quadriceps muscle.
Prehab also gives patients and parents a realistic understanding of the work ahead. ACL recovery is not simply a surgical event followed by rest. It is a structured process that requires consistency at home, in physical therapy, and during the transition back to running and sport-specific movement.
For school-aged athletes, the plan should account for practical issues that can otherwise become stressful: navigating stairs, carrying a backpack, sitting through classes, arranging transportation, and communicating restrictions to coaches. A well-organized plan lets the athlete stay connected to school and teammates while protecting the healing knee.
Rehabilitation Is the Return-to-Sport Treatment
The reconstructed ACL needs time to biologically incorporate and mature. At the same time, the athlete must regain motion, strength, balance, confidence, and movement quality. These pieces do not always return at the same speed.
Early rehabilitation usually emphasizes controlling swelling, regaining full extension, gradually improving flexion, and restoring normal walking mechanics. As healing progresses, therapy shifts toward single-leg strength, balance, landing mechanics, running, agility, and sport-specific drills.
A young athlete may feel good before they are truly ready for unrestricted play. That is why return-to-sport decisions should include more than a time frame. Many athletes need nine to 12 months or longer before returning to competitive pivoting sports. The exact timeline can vary, particularly after meniscus repair, with significant weakness, or when an athlete struggles to regain full motion.
Before clearance, the care team should evaluate several areas together:
- Full, pain-free range of motion with minimal or no swelling
- Strength recovery in the quadriceps, hamstrings, hips, and calves
- Quality of jumping, landing, cutting, and deceleration mechanics
- Functional testing that compares the surgical and nonsurgical legs
- Psychological readiness and confidence in sport movements
Passing a single strength test is not enough. A successful return means the athlete can meet the physical demands of their sport without compensating, hesitating, or exposing the knee to unnecessary risk.
The Risk of a Second ACL Injury
Young athletes have a meaningful risk of another ACL injury after reconstruction, either to the reconstructed knee or the opposite knee. This does not mean they should avoid sports forever. It means return-to-sport training and injury-prevention habits deserve the same attention as the operation itself.
Neuromuscular training can help athletes improve landing technique, hip and core control, balance, and awareness of knee position during cutting and jumping. These programs are particularly useful for athletes returning to high-demand field and court sports. Continuing them after formal therapy ends can support safer long-term participation.
Parents and coaches also play a role. Pressure to return for playoffs, showcases, or a college recruiting event can make it tempting to rush. The safer choice is to let objective recovery guide the schedule. Missing additional time is difficult, but returning before the knee is prepared can cost substantially more time later.

A consultation should give families a practical plan, not just an MRI interpretation. Ask whether the growth plates are open, which surgical technique fits the athlete’s development, what graft is recommended and why, and whether there is meniscus or cartilage injury to address.
